| Certificate of Child Health Examination: All children entering Pre-3, Pre-4, Kindergarten and Grade 6 are required to have a physical and a completed Certificate of Child Health Examination on file at the school. (This form is available at the school office and at your doctor's office, or download this form from the Illinois Department of Public Health.) Children entering Kindergarten must have a vision examination as well. For the 2011-2012 school year, the physical exam must take place after February 22, 2011. Both sides of the form must be completed. This includes a complete immunization record with a verifying signature, the health history completed and signed by the parent, and the physical exam portion signed by the person performing the examination. Children not meeting health requirements (physical and proper immunizations) are excluded from school beginning on October 13th.
Dental Examination: In addition, all children entering kindergarten and the second and sixth grades need to have a dental examination performed by a licensed dentist in accordance with the Illinois Department of Public Health requirements. Sports Physical: Children in grades 5-8 who plan on participating in athletics at St. Joseph School are required to have a sports physical on file for the school year. This physical must have taken place on or after February 22, 2011. REQUIRED PHYSICALS AND IMMUNIZATIONS ARE NEEDED PRIOR TO THE FIRST DAY OF SCHOOL! |
